Background of the Study
Risk management in healthcare refers to the identification, assessment, and prioritization of risks, followed by coordinated efforts to minimize, monitor, and control the probability or impact of unfortunate events. In hospitals, risks may arise from various sources including clinical errors, operational inefficiencies, financial mismanagement, and security threats. Risk management practices are designed to improve patient safety, enhance healthcare quality, and reduce operational costs. Nasarawa State, located in Nigeria's north-central region, has a growing healthcare sector but is challenged by various risks that threaten the effectiveness of healthcare service delivery. Hospitals in Nasarawa State face unique challenges including understaffing, limited financial resources, and a lack of adequate infrastructure, all of which heighten the need for effective risk management.
In the face of these challenges, healthcare facilities in Nasarawa State must adopt comprehensive risk management strategies to mitigate operational and clinical risks. Despite the growing recognition of the importance of risk management in improving healthcare outcomes, many hospitals in the state struggle with implementing effective risk management frameworks. Previous studies have shown that hospitals that do not adopt formal risk management practices are more likely to experience incidents that compromise patient safety and hospital performance. However, research on risk management practices within healthcare facilities in Nasarawa State is scarce. This study aims to fill this gap by investigating the risk management practices in hospitals across the state, evaluating their effectiveness, and proposing improvements to enhance the safety and efficiency of healthcare services.
Statement of the Problem
Effective risk management practices are essential to the success of healthcare facilities. In Nasarawa State, however, many hospitals still operate with limited or no formal risk management systems, which leaves them vulnerable to clinical errors, operational failures, and financial inefficiencies. These risks not only endanger patient safety but also harm the hospitals' reputation, financial stability, and overall service delivery. Without proper risk management strategies in place, healthcare facilities in Nasarawa State are at greater risk of facing challenges such as inadequate patient care, poor staff performance, and inefficient resource allocation.
Despite the crucial role that risk management plays in improving healthcare delivery, most hospitals in Nasarawa State operate without structured mechanisms to identify and manage risks. The absence of clear risk management policies, coupled with insufficient staff training, makes it difficult to effectively address issues such as medication errors, patient falls, and delays in service delivery. This study aims to investigate the current risk management practices in hospitals within the state, identify the challenges hindering the implementation of effective strategies, and provide recommendations to improve risk management in these institutions.

Scope and Limitations of the Study
The study focuses on hospitals in Nasarawa State, Nigeria, and will investigate the risk management practices in both public and private healthcare facilities. The research will examine risk management practices in clinical settings, administrative functions, and operational procedures.
Limitations include potential biases in self-reporting by healthcare workers and hospital management, as well as the challenge of accessing hospital-specific data on risk management practices. Time and resource constraints may also limit the number of hospitals that can be included in the study.
